I am so pleased that you are considering embarking on the journey towards health and wellness with me. My enrollment process is designed to allow us the opportunity to get to know each other and for you to develop a relationship with my staff that will support you through this empowering journey.

I want us to be aligned in our expectations and approach to your health care. As such, the application materials are thorough and the review process is selective. My intention is to insure a good fit for you, myself and the staff. The level of openness and receptivity that arises from this kind of partnership creates an ideal environment for growth and transformation.

After reviewing the material found throughout this web site, your first step is to call my office and speak with the enrollment staff at 949.574.5800 extension 1. During this initial interview you will learn about the process of becoming a new patient, my philosophy and the practice style. We will go over with you all the necessary forms you will complete as part of your application.

Upon completion of your interview you will receive an open house invitation and material on my services and fees. We endeavor to provide you with all the information you need to decide if moving forward is right for you.

I offer you the opportunity to speak with me directly at an open house where you'll meet the staff and get a tour of our lovely offices. Each conversation and interaction allows us to get to understand your desires and share the innovative and individualized style of medicine that I practice.

When you choose to become a new patient, you and I will have an ongoing and personal relationship. I take the time to get to know you, understand your concerns, goals and health aspirations. Together we partner to achieve your highest level of wellness. We will address your immediate medical concerns while also investigating the underlying subtle causes of chronic illness and supporting the body's innate ability to thrive.

Application Process Checklist

1. Call our office for an initial interview at 949.574.5800 extension 1.

2. Read the Welcome Letter.

3. Complete the comprehensive Medical History.

4. Read and sign the Consents.

5. Fill out requests for your prior Medical Records.

6. Copy your insurance card, front and back.

7. Review and complete the enrollment form.

8. Submit your completed application for review.

Scheduling

Once your application has been accepted, we will contact you to schedule your initial visit!

Bring To Your Initial Visit

1. Completed 3 day Food Diary.

2. All prescriptions, supplements, herbs, vitamins in original bottles.

3. Any relevant medical information not previously forwarded.
